How to Submit Your Forms Once you have completed your application or form, the City provides three convenient ways to return it:

  • Online: Complete and submit via our secure web portal below for instant delivery.

  • Email: Scan and send your completed documents to cityhall@centralcityky.com. (Please include the form name in the subject line).

  • In-Person/Mail: Drop off or mail your forms to City Hall at  214 North 1st Street Central City, KY 42330
